Every prehospital provider has responded to a "syncopal episode." But behind that familiar chief complaint lies a clinical spectrum that ranges from a benign vasovagal event to a lethal cardiac arrhythmia. The difference between the two often comes down to what you know to look for and the questions you know to ask.

This masterclass gives you a systematic, evidence-based framework for evaluating and managing syncope in the field. You will learn to move beyond "the patient fainted" and build a structured clinical picture using focused history, targeted physical examination, orthostatic assessment, and ECG interpretation. By the end of the course, you will be able to confidently differentiate high-risk cardiac syncope from lower-risk etiologies, make better transport decisions, and activate the right hospital resources at the right time.

  • Define syncope using both the clinical and pathophysiologic definitions and differentiate syncope from other causes of transient loss of consciousness.
  • Classify the major etiologic categories of syncope (cardiac, orthostatic, neurally mediated reflex, and syncope mimics) and list common causes within each category.
  • Perform a systematic syncope-focused history, including identification of preceding events, prodromal symptoms, bystander observations, and post-event features.
  • Identify high-risk features in the syncope history and physical examination that indicate a cardiac etiology requiring emergent evaluation.
  • Differentiate syncope from seizure using clinical features such as prodrome, duration, associated movements, and post-event recovery pattern.
  • Perform and interpret orthostatic vital signs and describe the significance of orthostatic hypotension in the context of syncope evaluation.
  • Interpret ECG findings relevant to syncope, including conduction abnormalities, arrhythmias, QT prolongation, and signs of structural heart disease.
  • Apply a structured approach to prehospital syncope management, including appropriate interventions for neurogenic, orthostatic, and cardiac syncope syndromes.
  • Evaluate clinical scenarios to determine appropriate transport decisions and hospital resource activation for syncope patients.
